The Diabetes Management Center provides an individualized diabetes plan of care for individuals with diabetes. It is comprised of board-certified endocrinologists, diabetes educators, dietitians and nurse specialists. The center collaborates with various health professionals, such as podiatrists, cardiologists, ophthalmologists, pharmacists, social workers and physical therapists. The Diabetes Management Center offers specialized services to assist individuals in the prevention and management of health complications through physician consultation, individual instruction, group education, medical nutrition therapy, community outreach and medication management services. In addition, the center administers a diabetes self-management education program that is recognized by the American Diabetes Association. It is a part of The William W. Backus Hospital, which is a nonprofit, acute care community hospital that was founded in 1893 and maintains a location in Norwich, Conn.
