Lee De La Cruz APRN started with C & C Health Care Services in 2025 and is very knowledgeable providing post op care for orthopedic surgeries, WOUND CARE, and formulating plan of care for continuity.

Nathan Fowler APRN has been with C & C Health Care Services since 1991. Nathan Graduated with his MSN in 2017. Nathan not only provides Primary Care he also works with Mental Health patients. The mission of C & C Health Care Services is to enhance the well-being of the community by providing comprehensive health care services including prevention, education, primary care, behavioral health, addiction services, & wound care services with a special emphasis on the military, medically underserved, and low-income populations. Our goal is to provide a "medical home" for the underserved by significantly expanding access to primary care, behavioral health, disease management, prevention, and educational services.

Ziad works for C & C Health Care Services in Wound Care. Academically, he completed his Master of Physician Assistant Studies at Touro University in 2021, where he honed his skills in patient assessment, diagnosis, and treatment planning.
